The study, titled "Exploring the e-cigarette and tobacco gateway hypothesis: a prospective replication study among adolescents in the Netherlands and Flanders," was published in BMJ.
The longitudinal study consisted of 2839 participants at baseline, 1276 at six months, and 1025 at twelve months.

“Analyses involved (1) the association of baseline e-cigarette use and tobacco smoking among never smokers; (2) the association of frequency of e-cigarette use at baseline and frequency of tobacco smoking at follow-up; and (3) the association between baseline tobacco smoking and subsequent e-cigarette use among non-e-cigarette users,” the researchers reported.

From the data collected, the researchers concluded that the relationship between vaping and smoking is bidirectional. “More frequent e-cigarette use at baseline was associated with more frequent smoking at follow-up. Baseline tobacco smoking was associated with subsequent e-cigarette use.”

The relationship between vaping and smoking

Meanwhile, several other studies investigating this relationship have identified explanations for this link. A recent study of high school students in the US has shown that adolescents who crave excitement and tend to experiment are more likely to use multiple illicit substances, including tobacco and/or vaping products.

Additionally, a research team from the University of Michigan, analyzing national data on tobacco product use among 8th, 10th, and 12th graders from 1991 to 2019, concluded that the "Gateway Theory" lacks foundation. The collected data indicated that daily smoking rates among 12th-grade boys increased by 4.9% per year between 1991 and 1998 but decreased by 8% per year between 1998 and 2006, and by 1.6% from 2006 to 2012.

Furthermore, there was a 17% annual decline in smoking between 2012 and 2019. This positive pattern was consistent among boys and girls across all grades and for Black and White adolescents, despite increased vaping rates among the same age group.

Study author Rafael Meza emphasized that with this data, there is no indication of a gateway effect. “So I think the good news is that the rapid increase in e-cigarette use has not resulted in a reversal of the downward trends in smoking and smokeless tobacco use, and if anything, those trends have accelerated.”
