US Study: Teen Smoking Declines as Vaping Rises

Led by Rafael Meza, a professor of epidemiology and global health at the University of Michigan in Ann Arbor, the research team analyzed national data on tobacco product use among 8th, 10th, and 12th-grade students from 1991 to 2019.

The collected data showed that the daily smoking rate among 12th-grade boys increased by 4.9% per year between 1991 and 1998, but decreased by 8% per year between 1998 and 2006, and by 1.6% from 2006 to 2012. Additionally, there was an annual decrease of 17% between 2012 and 2019, with overall daily smoking among 12th-grade students dropping to approximately 2% by 2019. This positive pattern was consistent among boys and girls across all grades and for Black and White adolescents.

"These are very low rates, and our goal from a public health perspective is to keep smoking at these rates or lower," said co-author of the study David Levy, a professor of oncology at Georgetown University in Washington, D.C.

The researchers observed similar decreases in adolescent use of cigars, cigarillos, and smokeless tobacco. Lead author of the study Rafael Meza said that while the decrease in tobacco use is encouraging, the increase in vaping is concerning. "While the increase in e-cigarettes is concerning and is something we need to address and combat, the decrease in other tobacco products, particularly cigarettes—the most concerning form of tobacco use—is accelerating."

"Gateway Theory" disproved once again

He added that the well-known "Gateway Theory," or the concern that vaping could act as a gateway to smoking, is clearly unfounded. "The overall decrease in smoking is something to highlight and celebrate, and it 'shows that it is a common pattern, that children are no longer smoking.'"

"But, on the contrary, what we found was that the decrease in smokers is accelerating," Meza said. "So I think the good news is that the rapid increase in e-cigarette use has not resulted in a reversal of the declining trend in smoking and smokeless tobacco use, and if anything, the trend has improved."

Similar patterns highlighted by other research

Similarly, the 2020 National Youth Tobacco Survey (NYTS) showed that e-cigarette use among adolescents decreased by one-third. Conducted between January 16 and March 16, the school-based survey showed that 19.6% of high school students (3.02 million) reported e-cigarette use, down from 27.5% (4.11 million) in 2019. Among middle school students, the number also decreased, from 10.5% (1.24 million) in 2019, to 4.7% (550,000).
