Boston University Expert Study That Debunks Vaping as a Cause of Heart Attacks

Titled, "Re-examining the Relationship Between E-cigarette Use and Myocardial Infarction: A Cautionary Tale," the current research refutes three previous studies that broadly linked vaping to an increased risk of heart attack, even among never-smokers. "Among never-smokers, e-cigarette use is not associated with an increased risk of heart attack," Siegel said, as quoted by Filter.
One of the flawed studies, titled, "Electronic Cigarette Use and Myocardial Infarction Among Adults in the US Population Assessment of Tobacco and Health," was published in the Journal of the American Heart Association in 2019 and co-authored by the renowned Stanton Glantz. The cross-sectional study used data from the 2013-2014 Population Assessment of Tobacco and Health (PATH) study baseline survey, looking for an association between vaping and heart attack.

The study authors found that people who vaped were more likely to report having had a heart attack in the past, and then concluded that e-cig use doubled a person's likelihood of having a heart attack.

It was actually heart attacks that "caused" vaping

Responding to claims on his blog page, “The Rest of the Story,” at the time, Siegel had pointed out “that the problem with drawing causal conclusions from these cross-sectional studies is that there is no way to know which came first: vaping or a heart attack.” As highlighted in response to similar research, Siegel points out that it is quite common for former smokers to switch to vaping after experiencing a heart attack and other health scares.

“If this were the case, then it is not vaping that caused the heart attack; rather, it was actually the heart attack that “caused” vaping. In epidemiology, we call this “reverse causation,” and it is a common limitation of cross-sectional studies, especially when they do not ask about the timing of exposure and outcome," he explained.

Sure enough, after several submissions by fellow academics, the journal indicated that the study failed to determine what came first, vaping or a heart attack. When asked to provide further data to determine the direction of this relationship, Glantz and his team failed to do so, leading to the retraction of the study.

Only vapers who also smoke are at higher risk of heart attack

Meanwhile, the new study by Siegel and his team compiled data from 175,546 respondents to the annual National Health Interview Survey from 2014 to 2019. They found that daily e-cigarette use was only associated with a higher incidence of heart attack among people who were also smoking regular cigarettes.

Furthermore, they found no evidence of an increased risk of heart attack among never-smoking vapers. These findings confirm Siegel's previous suspicions, that earlier studies had drawn their conclusions about the perceived relationship between vaping and smoking without accounting for smoking, which in this case is undoubtedly a very significant variable.
