Analyzing studies published in PubMed, Embase, Web of Science, the Wiley Cochrane Library database, and the 2018 Society for Research on Nicotine and Tobacco and Society for Behavioral Medicine conference abstracts, the current review aimed to investigate whether e-cigarette use was associated with smoking compared to non-users.
Out of 9199 results, the researchers included 17 studies in the meta-analysis. "There was strong evidence for a link between e-cigarette use among non-smokers and subsequent smoking (OR: 4.59, 95% CI: 3.60 to 5.85) when the results were meta-analyzed in a random effects model. However, there was high heterogeneity (I2 = 88%)," they reported.
Weak evidence for a link between vaping and cigarette consumption
The study explained that while the association between vaping among non-smokers and subsequent smoking appeared strong, the evidence was considered weak due to a number of factors. Firstly, data was collected through self-report measures of smoking history and not biochemically verified.
Secondly, none of the studies included negative controls that would provide stronger evidence of whether the association might be causal. Furthermore, much of the evidence also failed to consider the nicotine levels contained in the vaping products used by participants, making it impossible to conclude that nicotine was the driving mechanism behind this association.
Quitting smoking through vaping
Meanwhile, a recent cohort-intervention study aimed to determine whether smokers who tried to quit through vaping reported similar quit rates to those using commonly recommended smoking cessation aids.
The study, titled "Electronic cigarettes in standard smoking cessation treatment by tobacco counselors in Flanders: E-cigarette users show similar if not higher quit rates as those using commonly recommended smoking cessation aids," looked at the smoking cessation success of smokers trying to quit through vaping.
Participants were recruited by tobacco counselors and underwent 2 months of smoking cessation treatment (in groups). Participants were followed for 7 months after the quit date and were asked to complete questionnaires and undergo eCO measurements.
"One-third of all participants (n = 244) achieved 7-month smoking abstinence after the quit date, with e-cigarette users having higher odds of quitting smoking at the last session compared to NRT users," the researchers reported.
"People who smoke and chose e-cigarettes in the context of smoking cessation treatment by tobacco counselors showed similar if not higher smoking cessation rates compared to those who chose evidence-based smoking cessation aids (or none at all)," they concluded.
